Occupations Are More Than Self-Care Tasks

Occupations are not limited to Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Occupational therapy is a holistic, evidence-based rehabilitation profession that uses meaningful activities — physical, cognitive, social, emotional, spiritual, and environmental — to improve a person’s ability to participate in life.

The OTPF reminds us that occupation reflects everything people need, want, and are expected to do, encompassing the full spectrum of “doing, being, becoming, and belonging” (Wilcock & Townsend, 2004).
Within this broad occupational landscape, ADLs and IADLs remain critical indicators of functional independence and are often the first areas impacted by illness, injury, or aging.

Where do ADLs and IADLs land in the Activity Analysis Continuum?

The simple answer is these are types of occupations! They land in the distal end of the activity analysis continuum.

Activity of Daily Living

Instrumental Activity of Daily Living

  • Care of others (including selecting and supervising caregivers)

ADL Range of Motion Goals 

We often measure 'typical' range of motion as Within Normal Limits (WNL). However, for ADL goals, aiming for Within Functional Levels (WFL) can help patients achieve independence faster.

Key Point:

  • WNL = Normal range for most people
  • WFL = Enough range to perform daily tasks

Whenever you approach self-care management or other occupation-specific interventions, remember to first perform activity analysis of the occupation as it relates to their primary diagnosis and barriers.

  • Identify impacted occupations and holistic barriers

  • Use Activity Analysis

  • Design your plan of care using a "continuum" format to address all needs defined by your activity analysis

  • Explain this process to your client

 Example: If your patient has experienced a CVA and does not have functional mobility in her arm, and also has weakness in the unaffected arm….you know that you need to spend some time restoring strength and neuromuscular communication before you have her perform a self-care session. If you start with dressing, you run the risk of frustration, unachievable activity, causing an external locus of control for your client, and much more. This is just an example…but can be applied to any diagnosis and any patient. You must use activity analysis to break down the activities and build the patient back up from the most foundational components of occupational performance (body systems/functions/structures)
